Due to the variety of the insurance carriers and plans, we are unable to know which policies require a referral. Please contact your Primary Care Provider with the date and time of your appointment to ensure that you have a referral on file.
We ask that you please call their office 24 hours prior to your appointment and verify that a referral authorization has been put through to your insurance company. At the time of your appointment, if we do not have a referral on file, we will ask you to sign a waiver prior to seeing the provider.
